Document stating that a minor reached legal age of 21 lunar years of age
Requirements:
Presence of parent or guardian to confirm the application.
The concerned person must be at least 21 years old.
1. Copy of ID of the applicant (the minor).
2. Copies of IDs of the witnesses.
3. Copy of ID of parent or guardian.
